MPCA AmeriCorps HealthCorps
The MPCA AmeriCorps program (HealthCorps) will focus on connecting low income and medically underserved populations to primary health care through patient engagement activities. The goal of the program is to increase access to primary health care services through preventive care and chronic disease management, assisting Medicaid members in maximizing their health plan benefits, and addressing social determinants of health by eliminating barriers to care. The HealthCorps program will also focus on patient reengagement for gaps in care and preventive services that were postponed or disrupted by COVID-19. We currently have 9 members serving in 7 counties in Michigan.
AmeriCorps members will spend their service hours reaching out to individuals in the community that are not receiving primary and preventive services and connect them to care through community health centers.
Service activities include, but are not limited to:
-Reaching out to connect patients with primary care, including annual primary care visits, chronic disease management visits, or preventive services like immunizations and cancer screenings.
-Facilitating access to care by aiding with appointment scheduling, transportation assistance, and arranging interpretation.
-Connecting patients with supportive community services to address social needs and barriers to care.
-Reengaging patients who experienced delayed or disrupted care because of COVID-19 and promoting awareness of COVID-19 precautions and alternative access to care options, such as telehealth and curbside services.
