How to apply for Michigan Rehabilitation Services
Applying to receive services through Michigan Rehabilitation Services (MRS) is straightforward. Follow the steps below to get started.
To apply for services, you must complete and sign the MRS Application for Vocational and Employment Services.
If you are younger than 18 or have a court-appointed legal guardian, your parent or guardian must also sign the application. MRS staff can help you complete the application and provide reasonable accommodations upon request.

Download and complete the MRS Application for Vocational and Employment Services (MRS-3.0a), then submit it to an MRS office.
