Please be sure to complete the Supply Order Form, save a copy to your computer, and attach the populated form to an e-mail addressed to the MDARD-AIDForms@Michigan.gov.
To order by phone, call the MDARD AID Supply Line at 517-284-5800. Veterinarians will need to listen to the recording of required information and leave a voicemail.
Whether you are ordering by phone or online please allow one to three business days for your supply request to be processed and three to five business days for shipping.
EFFECTIVE DECEMBER 1, 2021: Private veterinarians must directly contact National Veterinary Services Laboratories (NVSL) to order tuberculin. Please visit www.aphis.usda.gov for order forms, and submit completed NVSL forms to NVSL_Concerns@usda.gov for fulfillment.
