Before applying for a license reinstatement hearing, check the status of your license by creating or logging into your online Secretary of State account, which you may access through Online Services under the "Driver’s License and ID" section.
To apply for a license reinstatement hearing:
- Complete and sign the Hearing Request Application.
- Send the Community Support Letter to 3-6 friends, family members or coworkers to complete (if you don't intend to have witnesses at your hearing).
- Find a qualified evaluator to complete the Substance Use Evaluation (if you have been arrested for any alcohol or controlled substance related offenses).
- Order a laboratory report from a 12-panel urinalysis drug screen with at least two integrity variables (such as specific gravity, creatinine, or pH level).
- Gather additional documents (if applicable):
- Ignition interlock report from your interlock provider that is dated within 30 days of submission
- DA-4P form from your doctor (if you are taking any medication to treat addiction, pain, or a mental or physical health condition that may impact your ability to drive safely)
- Certifications of completion or verification of participation from programs such as AA, other support groups, or individual counseling
To submit your Hearing Request Application online:
- Create or log into your Driver Appeal Integrated System (DAIS) account.
- Follow the prompts to upload all documents.
To apply for a license reinstatement hearing:
