7. Well notification form

The MiSTAR Well Notification Form is for the digital submission of certain required notifications to GRMD by Operators. Multiple wells and multiple notices may be reported per file. Current notifications include the Notification of Loss of Mechanical Integrity (Rule 324.811 (1) and (2)) and the Notification of Mechanical Integrity Testing (Rule 324.806 (2) and 808 (2)).

Data validation and field descriptions

When a form file is uploaded the application performs validation against both the header information and all corresponding detail records. Below users can find descriptions, hints and validation for the Well Notification Form:

 

Fields/Data Required? Validation Criteria and Description
Operator Name Yes Official operator/permittee name with EGLE.
Operator Number Yes EGLE Operator/Permittee Number. Must match operator uploading document.
14-digit API
Number
Yes Must be 14-digit number, must belong to company/permittee listed in header, and must be listed as a Class II well in the GRMD database. If this field shows an error indicating it is not Class II and you believe it should be please contact the GRMD UIC coordinator.
Well Name &
Number
Auto This field is auto-populated based on the API number.
County Name Yes Full county name for location of well.
Township Name Yes Township name for location of well.
Notification Type Yes Choose from: ‘Notice of MIT Scheduled’ or ‘Notice of Loss of Mechanical Integrity’.
Applicable Date Yes MM/DD/YYYY. Date related to the notice being reported (not the date of submittal).
Applicable Time No HH:MM in military time. Time related to the notification being reported (not the time of notification submittal). For example- a Notice of MIT Scheduled would give the time the MIT is scheduled for.
Description No Provide additional details and pertinent contact information for questions.
