8. Document uploads
The MiSTAR Document Uploads form allows users to upload Class II UIC related documents. Operators can digitally submit and/or view the following documents: Injectate Analysis Annual, Injectate Analysis Other, Commercial Sources New, Commercial Sources Quarterly, Correspondence, MIT Gauge Records, MIT Chart, and MIT RCOWS. Operators can also submit single well documents or multiple wells on a single report.
Create new document uploads
In the second version of MiSTAR Document Uploads are available via the Create Submission button on the main Submissions Page.
The following options are displayed. Fields with an asterisk are required. The following document types can be uploaded: .doc, .docx, .xls, .xlsx .pdf, and .tiff. Maximum file size is 20 MB.
The Wellbores section of the submission detail page is required and allows for either single or multiple well selections. Click the +Add Record button to select a new wellbore to associate to the document submittal. Wells can be searched using the well name or API Number. Select additional wellbores as needed.
The Documents section of the submission detail page is where documents can be uploaded. Click the +Add Document button.
Current Document Types that can be uploaded include:
- Injectate Analysis Annual
- Injectate Analysis Other
- Commercial Sources New
- Commercial Sources Quarterly
- Correspondence
- MIT Gauge Records
- MIT Chart
- MIT RCOWS
Users may type in the dropdown box to find the correct type. The Relevant Date field is optional but encouraged. After all required fields are filled in and a file is attached, the Save button changes from gray to blue and the document is uploaded to the submission.
Click the Validate button to ensure any errors have been corrected and activate the Submit for Review button. Then click the Submit for Review button to submit the documents for approval by GRMD.
