Refrigeration Facility Relicensure

• Complete a relicensure application for Refrigeration Facility online in MiCLEAR. 
• You must have a physical location in Michigan to obtain a Refrigeration Facility license. 
• The applicant must be of good moral character. This includes the owner, operator, officer, director, partner, member, or manager of the applicant.
• The applicant must disclose any criminal history.

• A Refrigeration Facility must designate a manager with an active Mortuary Science license. 
• The designated manager shall supervise the handling and sheltering of a dead human body at the Refrigeration Facility. The manager shall not otherwise engage in the practice of Mortuary Science while acting as a manager for a Refrigeration Facility.
• The designated manager cannot manage more than one Refrigeration Facility.
• The designated manager of a Refrigeration Facility shall not reside more than 100 miles from the Refrigeration Facility. 

• If applying as a corporation, upload the Articles of Incorporation; or 
  • If applying as an LLC, upload the Articles of Organization; or 
  • If applying as a sole proprietor, upload the “Doing Business As” document from the county or local government. 
  • If using a name other than the legal entity name, upload the Certificate of Assumed Name filed with Michigan’s Corporation Division. 

• Upload documents related to any criminal conviction, if applicable.

An inspection will be scheduled once all application requirements have been met.
