Relicensure Information

  • Required Documents

    Submit "Mortuary Science Courtesy Relicensure Application" through the MiCLEAR portal.

    In order to consider an application complete and to avoid delays of processing, the following documents must be uploaded as part of the application: 

    • Copy of your active embalmer, funeral director or mortuary science license from Ohio, Indiana or Wisconsin.
