Mortuary Science Funeral Establishment Relicensure

Submit "Mortuary Science Funeral Establishment Relicensure Application" through MiCLEAR. 

In order to consider an application complete and to avoid delays of processing, the following documents must be uploaded as part of the application: 

• Entity documents as filed with the State of Michigan, Corporations Division or if a Sole Proprietorship, as filed with your local County Clerk’s Office.

• Need to provide active Prepaid Funeral/Cemetery Contract Seller/Provider Registration Number.

• Need to provide active Medical Waste Facility Registration number for the location you are applying for license.

• If applying for manager of two funeral establishments, must submit the Waiver Request.

Once your application and required documents submitted have been reviewed and determined to meet the requirements of licensure, a Licensing Inspector will contact you to schedule your Licensing Inspection.
