Frequently Asked Questions

  • Enter the year in which the required services were completed.

  • Check the box for the 6-month period in which the required services were performed.

  • Yes, even if the number is "0".

  • If this is not your first report submitted, review the most recent approved by the Department. Enter the total number of services you previously reported related to embalming and assisting a licensee in embalming. If this is your first report, enter "0".

  • Enter the total number of services you completed during this 6-month rating period related to embalming and assisting a licensee in embalming.

  • Add the number in the field titled "Previously Reported" and from the field "Completed This Period" and enter that number in this field. If this is your final report, the numbers in this column should not be under "25".

  • Please email your completed report to CSCLonline@michigan.gov. Include your license number in the subject line of your email to the Department. Please do not submit the report by U.S. Mail. (This process will be changing soon and will require you to upload your report into MiCLEAR. Once this change has been implemented, you will be notified by email.)

  • Your report will be reviewed by staff and either marked as "approved" or "not approved" and will be uploaded into your MiCLEAR account for you to view. If your report was marked as "not approved",  you will receive an email detailing the reason(s) it is not approved.

  • MCL 339.1808(2), states the Embalming and Resident Trainee Rating Reports are due semiannually on January 15 and July 15 showing the work which the trainee completed during the 6 months preceding the first of the month in which the report is made. Reports received after January 15 and July 15 are considered to be late submissions.

  • After the completion of your Resident Trainee time and service requirements, and all the requirements for the Mortuary Science License application have been met, the Department will email you with instructions on how to register for the exam. You cannot take the exam while licensed as a Resident Trainee.

  • Per Rule 339.18923, for at least 12 months (time requirement) and until all required services have been performed. Your time begins on the date you obtained your Resident Trainee license. If you perform services when your license is not in an "active" status, you will not receive credit for the services performed. You are allowed a one-time renewal of your Resident Trainee license.

  • You would need to submit two Embalming and Resident Trainee Rating Reports. One report would be completed by the initial sponsor and one would be completed by the new sponsor. Be sure that you submitted a "Change of Sponsor" application online in MiCLEAR to update the new sponsor once that changed.
