Name Change Information

  • As the Branch Manager, my legal name has changed. How do I change my name with the Department?

    Please email the Licensing Division at CSCLOnline@michigan.gov and request a "Legal Name Change". Include the following in your email:

    • Full name currently on record for qualifying officer and branch manager. 
    • New full legal name of branch manager. 
    • License numbers for agency and branch.
    • A copy of your legal documents showing the name change of the branch manager.

    The Department will notify you if additional information/documentation is needed. 

  • My branch has a legal name change. How do I change the branch name on my license?

    Please email the Licensing Division at CSCLOnline@michigan.gov and request a "Legal Name Change". Include the following in your email:

    • Full name of your agency and branch as printed on the licenses. 
    • New full legal name of the agency and branch. 
    • License numbers for the agency and the branch.
    • A copy of your legal documents showing the name change.

    The Department will notify you if additional information/documentation is needed.
