Certificate of Insurance or Surety Bond

  • Log into your MiCLEAR account and find your 10-digit license number that begins with "3601" under the "Licenses" tab. Click directly on the words "Additional Actions" and select "Update Insurance" and then follow the prompts. 

    Certificate of Insurance or Surety Bonds are required to be uploaded into MiCLEAR. Please notify your insurance agent and ask them to discontinue mailing and emailing to the department.  

  • Must meet any of the following requirements listed in MCL 338.1059 (4).

    • Surety bond
    • In the principal amount of $25,000.00.
    • Includes a bond number and effective date.
    • Must be in the name of the licensee as licensed by the Department.
    • Must be payable for the benefit of the people of the state and a person injured by the willful, malicious, and wrongful act of the licensee or any agents or employees of a licensee may bring an action on the bond or insurance policy in his or her own name to recover damages suffered by reason of the wrongful act.  
    • Must have name of Surety Company.
    • Includes signature of Owner/President/License Holder and a Witness with date.
    • Includes signature of Surety and a Witness with date.

     

    • In lieu of a bond, a policy of insurance
    • Issued by an insurer authorized to do business in this state that names the licensee and the state as coinsureds.
    • In the amount of $25,000.00 for property damages, $100,000.00 for injury to or death of 1 person, and $200,000.00 for injuries to or deaths of more than 1 person arising out of the operation of the licensed activity. 
    • Must be in the name of the licensee as licensed by the Department.
    • The State of Michigan must be listed as the Certificate Holder and additional insured. 
    • A signature is required for the Authorized Representative.
    • Includes a policy number with policy effective and expiration dates.
    • Includes contact information of the insurance provider.
