Independent/Political (PAC) Committee Forms and Instructions

These forms are now updateable.  Please fill in the necessary fields as needed and Print.  These forms are used for State level (February, April, July, October Quarterlies, Dissolution and amended campaign statements) and local PACs (Pre/Post Election), Dissolution and amended campaign statements on paper.

As soon as a "person" or group receives or spends $500.00 or more in a calendar year the MCFA requires the "person" or group to form and register a committee.  Once the $500.00 threshold has been met the PAC has 10 calendar days to form and register a Statement or Organization with the appropriate filing official.  The statement of organization form is also to keep up-to-date contact information, and request a Reporting Waiver if the committee does not expect to spend or receive more than $1,000 (Review Appendix C for requirements of obtaining a reporting waiver).

 

** Committees required to file electronically cannot use these forms.  Review Appendix D for more information.

 

Forms and Instructions: 

· PAC Cover Page - Each Campaign Statement must include a complete cover page with each filing.

· PAC Summary Page - Used to report a quick summary of financial activity during reporting coverage period.

· PAC Itemized Contributions Schedule 2A - Used to report direct contributions or loans of money from a person regardless of amount.

· PAC Other Receipts Schedule 2A-1 - Used to report interest, misc., refunds, rebates, returned/uncashed checks or loans from banks.

· PAC In-Kind Contributions Schedule 2-IK - Used to report contributions of goods or services from the candidate, person or other committees.

· PAC Expenditures Schedule 2B - Used to report direct expenditures of money of $50.01 or more in value.

·PAC Itemized Independent Expenditures Schedule 2B-1 - Used to report independent expenditures.

· PAC Itemized In-Kind Expenditures Schedule 2B-2 - Used to report fair market value of goods or services donated to other committees or organizations.

· PAC Get-Out-The-Vote Activities Schedule B-G - Used to report expenditures made for election day get out the vote activities.

· PAC Debts and Obligations Schedule 2E - Used to report funds owed by or owed to the committee.

·PAC Fundraiser Schedule 2F - Used to report information on fund raisers held by the committee.

· PAC Forms and Instructions Booklet - Complete booklet of all campaign finance forms and instructions.
