These forms are now updateable. Please fill in the necessary fields as needed and print. These forms are used to file Pre-Election, Post-Election, Annuals, Dissolution, Quarterly and amended campaign statements on paper.
A candidate covered by the MCFA registers the committee by filing a Statement of Organization form. The statement of organization form is also used to keep up-to-date contact committee information, and request a Reporting Waiver if the committee does not expect to spend or receive more than $1,000 (refer to Appendix C for requirements of obtaining a reporting waiver).
