  • Below are instructions that allow an individual to generate “real time” data in a searchable format.

  • Adult Foster Care (AFC) Homes and Homes for the Aged (HFA)

  • Health Care Providers and Substance Use Disorder Programs

    Visit our Verify a License website. Select the “Facility License Type” of interest. This site defaults to active facilities under “Status”, but you can also select “closed and lapsed” if interested. Once you have selected the applicable “Facility License Type” and “Status”, select “Search”.  This will generate a list of applicable providers. You can then download this list in a searchable format by selecting “Download CSV”.
