The Michigan Department of Licensing and Regulatory Affairs (LARA) will never ask you to provide credit card numbers or other personal information by phone, text, email, or mail. LARA has identified fraudulent communications impersonating department staff. These scams may include requests for payment or personal information. Please do not respond to these messages and report any suspicious activity to LARA‑Safety@michigan.gov. MAPS Users
PRACTITIONERS, PHARMACISTS AND DELEGATE USERS

MAPS enables practitioners to determine if patients are receiving controlled substances from other providers and to assist in the prevention of prescription drug abuse. Registration to MAPS is easy and only takes a few minutes. MAPS is available 24/7 to request MAPS reports.
Registration
To register to MAPS, go to https://michigan.pmpaware.net and click the “Create an Account” link.
Accounts are for individual use only, and credentials shall not be shared. Organizational/shared accounts are not permitted, and your email address should be specific to you and not a shared email/group account.
For licensed healthcare professionals, please have the following credentials readily available as they are required for registration:
- Drug Enforcement Administration (DEA) number
- For pharmacists and pharmacist delegates, the pharmacy DEA number
- Professional license number
- Controlled substance number (as applicable by user role; noted as 'CS ID' in MAPS registration)
- National Provider Identifier (NPI) number (as applicable by user role)
Login & Account Support
Account credentials are validated at registration and subsequently on a daily basis. Common login issues include but are not limited to:
- A credential (e.g., DEA number, license number) status change occurred, causing your account to deactivate.
- Your password may need to be reset. Note all MAPS account passwords are valid for 90 days. To reset your password, go to the login page at michigan.pmpaware.net and click the "Forgot Password" link.
- Your account may be locked. After too many invalid login attempts, your account will be locked for 20 minutes and cannot be unlocked by a MAPS Administrator or Bamboo Health Support.

LAW ENFORCEMENT, GOVERNMENT AGENCIES, AND BENEFIT PLAN MANAGERS

MAPS allows law enforcement, government agencies and benefit plan managers to register online and submit requests in accordance with the Public Health Code. MAPS requires these users to submit a completed registration form to attach to their online registration. Registrations and data requests are reviewed on a case-by-case basis, during normal business hours.

To register under a non-healthcare user role, a registration form is required and can be found under Important Documents below. The form shall be completed and signed by you and your supervisor and uploaded during account registration when prompted. Please note only physical or encrypted electronic signatures are acceptable.
