Prepaid Funeral and Cemetery Contract Seller/Provider Registration	The company is formed at the state level as a corporation, limited liability company, etc., or the County level as a Sole Proprietorship. Please contact the Corporations Division for more information on how to form a company in Michigan or your local your local County Clerk’s office for a Sole Proprietorship.
