How to Submit an Annual Report in MiCLEAR

Important:

For best results, use a desktop or laptop computer. You may have trouble using a mobile device (i.e., Smartphone, iPad, etc.).

Please make sure you have registered with a MiCLEAR account and have linked your license before starting this process. If you have not registered for an account, please visit How to Register for an Account. If you have not linked your account, please visit How to Link your License.

Annual Reports are for Prepaid Funeral Providers License, Cemetery Registration and Crematory Registration.

Welcome to MiCLEAR

Getting Started

1. Visit www.michigan.gov/MiCLEAR

2. Click on "Login to MiCLEAR"

Sign In - Username and Password

Step 1: Sign In

1. Enter Username or Email

2. Enter Password

3. Click on "Sign In"

Step 2: Additional Actions

  1. Click on the “Licenses” tab.
  2. Next to your license number, you will find the words “Additional Actions”. Click directly on the words “Additional Actions”. Do not click on your license number. 

Clicking on the words “Additional Actions” will take you to where you will select an Amendment Type.

Step 3: Amendment Type

  1. Click on the radio button next for either "Prepaid Funeral Annual Report”, "Cemetery Annual Report", or "Crematory Annual Report", as appropriate
  2. Click on “Continue”

Step 4: Report Year

Click on the drop down next to the report year and choose the year for the annual report being filed.

Step 5: Attachments

Step 5a: Add a Document

Documents can be uploaded by clicking the “Add” button.

Click on "Add".  You will need to find the desired saved document on your computer (double click on it.)

Clicking on the document on your computer will upload the file.

When the blue status bar says "100%", click on “Continue”.

Step 5d: Attachment Type and Save

1. Use the drop down arrow and click on "Annual Report."

2.Click on "Save"

Once a green checkmark appears with the statement, "The attachment(s) has/have been successfully uploaded,” click on the “Continue” button to proceed to the “Review” section.

Step 6 Review

Review the Record Type, Report Year and Attachments for accuracy.

  1. Record Type: Review that you are filing the correct Annual Report (Prepaid Funeral, Cemetery, or Crematory).

2. Report Year: Review the year is correct. If it needs to be updated, click on edit to make updates.

3. Attachments: Make sure the document is attached and correct. If it is not,  click on edit to make updates.

4. Once everything has been reviewed, click on "Continue."

  1. The record issuance screen will appear, letting you know that your application has been submitted.  If you do not see confirmation that your application has been successfully submitted, please contact us at (517) 335-5237 or LARA-CSCL-Securities-Audit@michigan.gov. (Records beginning with TMP have not been submitted for review.)
  2. Please note your Record Number for this transaction. You will need this number to check the status of your application. 

Once successfully submitted, your Annual Report is placed in your responsible worker’s queue to review for completeness and accuracy. Your submission of the annual report does not mean it is acceptable and approved. If anything further is needed, you will be contacted.
