Licensing and Regulatory Affairs
This page is designed to assist providers through the state licensure process for Substance Use Disorder (SUD) programs, as well as provide additional resources to maintain compliance.
State Licensing is required for all SUD programs. To initiate an SUD program, a state licensure application, documents, and applicable licensing fee are required. In addition, Appendix I is required for SUD programs requesting to offer residential services and Appendix II is required for SUD programs requesting to offer Outpatient Treatment of Opioid Dependence or Methadone.
