State Licensure

    • Change of ownership
    • Change of program name
    • Change of location of program
    • Change of contact information
    • Change of program director
    • Addition or removal of service categories

Licensing Fees

  • The following licensing fees apply to the licensure of SUD programs:

    • Initial licensing fee: $500
    • Annual licensing fee:$500
      • Renewal notices are sent in June of each year
      • License renewals must be paid online no later than July 31st
