    When contacting the Licensing Division, you may be asked to provide your license/application number and cemetery name. Your license/application number will begin with the number "220".

  • Registration Type: Description:
    Cemetery The company is formed at the state level under the Cemetery Corporations Act. Please contact the Corporations Division for more information on how to form a company in Michigan.
    Crematory The company is formed at the state level under the Cremation Companies Act. Please contact the Corporations Division for more information on how to form a company in Michigan.
    Fee Exempt Cemetery The company is formed at the state level under the Cemetery Corporations Act or Rural Cemetery Corporations Act. Please contact the Corporations Division for more information on how to form a company in Michigan.

    You want a new registration and have never been issued this registration type in Michigan.

    Change of Control

    You are purchasing or taking ownership of an actively registered cemetery.

    Renewal

    Your active registration is about to expire, and you wish to continue registration.

    The registration is considered active until the expiration date printed on the registration.

    Crematory $10 per day beginning 6/2 6/2 annual until renewed.
