Employee Identification (ID) Card Information

  • Can my employees carry an identification card?

    Yes, if the employee identification card template is approved by the Department.

    Per MCL 338.1064, the department will issue an identification card to the principal license holder. The identification card is wallet sized and is attached the license print.  The licensee is responsible for the maintenance, custody, and control of the identification card and shall not let, loan, sell, or otherwise permit unauthorized persons or employees to use it.

  • How do I obtain approval for my employee identification card template from the Department?

    Please email the Licensing Division at CSCLOnline@michigan.gov and request an “Employee Identification Card”. Include the following in your email:

    • Full name of licensee as printed on the license.
    • The license number.
