Private Security Guard Sole Proprietor Employee Roster

Employee Roster (Quarterly Report) Information

  • The Department provides an Employee Roster Form that can be found here. All employees for the agency and any branches should be combined on one Employee Roster form. 

    For questions regarding the hiring of employees by a licensee, see MCL 338.1067.

    Employee Rosters must include the following information, or they will not be accepted:

    • Agency and Branch license number (if applicable)  
    • Printed name of the person preparing the report, their signature, their title, and the date of signature
    • Quarter covered by the Employee Roster
    • Alphabetical listing of all persons employed by the agency
    • Employee date of birth
    • Employee hire date
    • Employee termination date
    • Total number of employees for quarter (Will always be at least one)
    • Sworn Statement as Follows: I affirm that I am authorized to act on behalf of the agency, and I verify that the information is true, complete, and accurate to the best of my knowledge and belief.
  • Employee Rosters (Quarterly Reports) are due on the following dates:

    • First Quarter (January-March) Employee Roster is due by April 15 each year.
    • Second Quarter (April-June) Employee Roster is due by July 15 each year.
    • Third Quarter (July-September) Employee Roster is due by October 15 each year.
    • Fourth Quarter (October-December) Employee Roster is due by January 15 each year.

    Please do not submit your Employee Roster until the quarter you are reporting on has ended but before the due date.

  •  To upload employee rosters:

    1. Log into your MiCLEAR account and find your 10-digit license number that begins with "3801" under the “Licenses” tab.
    2. Click directly on the words “Additional Actions” and select “Security Guard Quarterly Report”.

    If you need to upload multiple quarters, you must upload each quarter separately. There cannot be multiple quarters uploaded under one record.
