Application Process for State Licensure
This page is designed to assist providers through the state licensure process for Substance Use Disorder (SUD) programs, as well as provide additional resources to maintain compliance.
State Licensure
State Licensing is required for all SUD programs. To initiate an SUD program, a state licensure application, documents, and applicable licensing fee are required.
Existing SUD programs may request changes to the program by completing the application for the following actions:
- Change of ownership
- Change of program name
- Change of location of program
- Change of contact information
- Change of program director
- Addition or removal of service categories
Licensing Fees
The following licensing fees apply to the licensure of SUD programs:
- Initial licensing fee: $500
- Annual Licensing fee: $500
- Renewal notices are sent in June of each year
- License renewals must be paid online no later than July 31st
