September 13, 2019 – The Dept. of Licensing and Regulatory Affairs (LARA) has summarily suspended and issued a notice of intent to revoke the child care license of Sally Ann Hulbert (128 Maplelawn Street SE, Kentwood) for violations related to appropriate care and supervision.
LARA’s actions resulted from a September 12, 2019, complaint investigation conducted at the family child care home. The investigation found violations of the Child Care Organizations Act and administrative rules regarding family and group child care homes. LARA took emergency action to protect the health, welfare, and safety of children.
The summary suspension order prohibits Hulbert from operating a family child care home at 128 Maplelawn Street SE, Kentwood, or at any other address or location. Accordingly, she may not accept children for care after that date and time. The order also requires the licensee to inform all parents of children in her care that the license has been suspended and that she can no longer provide child care.
The licensee has the right to appeal this suspension. This case will be forwarded to the Michigan Office of Administrative Hearings and Rules (MOAHR) for a hearing date.
