Name Change Information

As the Branch Manager, my legal name has changed. How do I change my name with the Department?
Please email the Licensing Division at CSCLOnline@michigan.gov and request a "Legal Name Change". Include the following in your email:

Full name currently on record for qualifying officer and branch manager.

New full legal name of branch manager.

License numbers for agency and branch.

A copy of your legal documents showing the name change of the branch manager.

The Department will notify you if additional information/documentation is needed.
My branch has a legal name change. How do I change the branch name on my license?

New full legal name of the agency and branch.

License numbers for the agency and the branch.
