Licensing Division Virtual Appointment Request
You may request a virtual appointment with a staff person from the Licensing Division to discuss any questions you may have with a filed application, renewal, non-profit Special License, or other license or permit questions by using the form below. Once you have submitted your virtual appointment request, a staff person from the Licensing Division will contact you within 24 hours of your request (except on weekends).
An appointment is not required if you have general licensing questions. Instead, you may call and speak with a staff member directly by calling 866-813-0011. Our phone lines are open Monday - Friday from 8:00 am to 5:00 pm.
