October 12, 2017 - The Dept. of Licensing and Regulatory Affairs, Bureau of Community and Health Systems (BCHS) issued an order of summary suspension and notice of intent to revoke the license of an adult foster care small group home licensee, Kristen Martell at 1405 Forrester Drive SE, Grand Rapids, MI. This action resulted from a recent investigation of a complaint of the adult foster care small group home.
The October 5, 2017 complaint investigation found violations of the Adult Foster Care Facility Licensing Act and/or administrative rules regarding the licensee’s financial capability, medication administration, background checks, staff training, and resident protection. BCHS took emergency action to protect the health, welfare, and safety of vulnerable adults.
Effective 6 p.m. on October 6, 2017, the summary suspension order prohibits Kristen Martell from operating an adult foster care small group home at 1405 Forrester Drive SE, Grand Rapids, MI, or at any other address or location. Accordingly, she may not accept physically handicapped, developmentally disabled, mentally ill or aged adults for care after that date and time. The order also requires Ms. Martell to inform all of the guardians of adults in her care that the license has been suspended and that she can no longer provide adult foster care services.
Michigan law defines an adult foster care small group home as “an adult foster care facility with the approved capacity to receive 12 or fewer adults to be provided with foster care.”
A licensee has the right to appeal this suspension. This case will be forwarded to the Michigan Administrative Hearing System (MAHS) for a hearing date.
