LARA Summarily Suspends Kent County Family Child Care Home - Dana Adamczak

January 23, 2017 - The Michigan Department of Licensing and Regulatory Affairs, Bureau of Community and Health Systems (BCHS) issued an order of summary suspension and notice of intent to revoke the certificate of registration of a Kent County family child care home provider, Dana Adamczak, License #DF410296842, 4115 Sawkaw Drive, NE, Grand Rapids, Michigan 49525. This action resulted from a recent investigation of a complaint against the family child care home.
The January 3, 2017, complaint investigation found violations of the Child Care Organizations Act and/or administrative rules regarding caregiver suitability, appropriate care and supervision, and assistant caregiver training. BCHS took emergency action to protect the health, welfare, and safety of the children.
Effective 6:00 p.m., January 4, 2017, the summary suspension order prohibits Dana Adamczak from operating a family child care home at 4115 Sawkaw Drive, NE, Grand Rapids, Michigan 49525, or at any other address or location. Accordingly, she may not accept children for care after that date and time. The order also requires Ms. Adamczak to inform all of the parents of children in her care that her certificate of registration has been suspended and that she can no longer provide child care.
Dana Adamczak has held a certificate of registration to operate a family child care home since August 27, 2008. The certificate of registration was for 6 children.
Michigan law defines a family child care home as “a private home in which 1 but fewer than 7 minor children are received for care and supervision for compensation for periods of less than 24 hours a day, unattended by a parent or legal guardian, except children related to an adult member of the family by blood, marriage, or adoption. A family child care home includes a home in which care is given to an unrelated minor child for more than 4 weeks during a calendar year. A family child care home does not include an individual providing babysitting services for another individual.”
A registrant has the right to appeal this suspension. The case will be forwarded to the Michigan Administrative Hearing System (MAHS) for a hearing date.
