Become a Registered Agency

To become a registered agency, you must:

Submit calibration reports for the standards that will be used in Michigan.

These are the certificate(s) showing all your agency’s standards and test equipment that are certified by Michigan’s Metrology laboratory or equivalent laboratory.

 Certification(s) must be within the past 2 years.

These must be submitted by mail with the agency’s application to:

Submit a receipt of payment from the cashier’s office.

  • Biennial registration fees are $300 for the service agency.
  • Please make check/money order payable to the State of Michigan.
  • Submit the check or money order with the application to:
  • For credit cards, email the completed application to the MDOT Cashier at MDOT-ASCCashier@michigan.gov then call them at 517-241-6001 to make a payment.
  • The application must be emailed immediately prior to making the payment.  E-mail the receipt and application to hartmanr4@michigan.gov.

Pass the Michigan Registration Exams

To become a Michigan Registered Agency, you are required to have at least one technician pass the Michigan Registration Exams:

  • Act 283
  • NIST Handbook 44, General Code
  • Device specific exam(s)

Service Technician Registration

To schedule a test date, submit the Serviceperson Registration Application, a Service Person Test Date form, and receipt to hartmanr4@michigan.gov.

You will receive an e-mail confirmation for the assigned test date.

Cost is $100 per serviceperson; please see above for payment instructions.

Calibration reports are only submitted with the agency application.

Test Report/Calibration Report Requirements

Michigan Weights and Measures does not approve or provide a standard calibration report.

Each registered agency must create their own report.  The report must be an official agency document used by all registered technicians in Michigan. The report must be on standard-sized paper (no legal-size documents).

The minimum information that must be on the test/calibration report:

  • Registered service agency information
    • Agency name, address, city, state, and zip code
    • Letterhead or with an agency logo
  • Service location information
  • Establishments name, address, city, and zip code
  • Date of service
  • Device identification
  • Make and model number 
  • As found and as left test data
  • Security seal information
  • For meters, you must show the amount pumped.
  • For scales, you must show the increasing load test, the decreasing load test, and the shift test.
  • Printed name(s) and signature(s) of the technician(s) that completed the work.

Security Seals

All agencies will be required to utilize security seals unless the device is equipped with an audit trail.

They must have unique markings (letters or Roto seal® with a serial number) that identify the company.

  • Companies creating new seals must submit in writing a description of the seal to Weights and Measures.
  • Companies using Roto seals® will need permission from Weights and Measures and the serial number from the Roto seal® will need to be on the Place in Service report.
  • Companies may not use a crimping tool (plier marks) to secure security seals.
