March 29, 2021 - The Dept. of Licensing and Regulatory Affairs (LARA) has summarily suspended and issued a notice of intent to revoke the adult foster care small group home license of Aina Acres AFC LLC, located at 5508 Mt. Vernon Avenue, Portage.
LARA's actions resulted from a March 24, 2021 complaint investigation conducted at the adult foster care small group home. The investigation found violations of the Adult Foster Care Facility Licensing Act and administrative rules related to mistreatment of a resident, physical abuse of a resident, and caregiver suitability. LARA took emergency action to protect the health, welfare, and safety of vulnerable adults.
Effective March 26, 2021, the summary suspension order prohibits Aina Acres AFC LLC from operating an adult foster care small group home at 5508 Mt. Vernon Avenue, Portage, or at any other address or location. Accordingly, Aina Acres AFC LLC may not accept physically handicapped, developmentally disabled, mentally ill or aged adults for care after that date and time. The order also requires the licensee to inform all the guardians of adults in their care that the license has been suspended and that it can no longer provide adult foster care services.
