Performance Reporting

  • No. As a reminder, all performance reporting for FY20 will need to follow the new format (quarterly for construction and annual for non-construction).

  • You must submit two different types of report for this grant. For the equipment purchase an annual report is required on form 5100-140 each year the grant is open and is due Oct. 31. The construction project must be reported quarterly on Form 5370-1 until construction is complete and the project is accepted.

  • Since you have a contractor installing the equipment this is a construction project must be reported quarterly on Form 5370-1 until construction is complete and the project is accepted.

  • Refer to the instructions included with the form, which describes each milestone that needs to be identified. 

  • No as long as you can report all the key milestones for each project on one form. If they do not fit, you will need to use two forms. 

  • If they are completed with one prime contractor for both projects, you will need to submit one 5370-1 form. If they are two separate contractors and contracts, you will need to submit two reports, one for each contractor/contract.
