Filing bond and proof of insurance electronically

Proof of insurance and/or the permit performance bond must be submitted electronically via the MDOT E-Performance Bond and Certificate of Insurance online application system. Marsh forms and Acord forms are not acceptable.

Surety and/or insurance agents are required to contact our office in order to register and obtain access to this application. If you have not previously registered or have questions, email MDOT-ePerformanceBondCOI@Michigan.gov.
