Request for Confidential Children’s Protective Services Records
Child Protective Services records are confidential as outlined by the Michigan Child Protection Law and Services Requirements Manual 131. Records may be released to certain individuals as outlined by the Michigan Child Protection Law. Timeframes for receiving the records vary. Confidentiality of information in the Child Protective Services record is protected by the Michigan Child Protection Law. Anyone who violates this protection is guilty of a misdemeanor and is civilly liable for damages (MCL 722.633(3), as amended, MCL 722.621 et. Seq.).
To make a request, please complete the following:
- Completed a MDHHS-6184, Request for Child Protective Services for Records form.
- Attach all required verifications.
- Mail or hand deliver MDHHS-6184 form to any MDHHS office for processing.
If the information is not complete, the request will not be processed.
Please note that requests may take 14-30 days to process. This may be extended based upon the volume of records requested.
