Welcome to WIC
An in-person WIC appointment is the first step. Your visits to our WIC clinics let us check you and your child's growth and health. It also helps us give your family the best resources and referrals.
Our in-person services include:
- Weighing and measuring to ensure good growth and development
- Checking blood levels related to iron
- Providing nutrition counseling to you and your family's questions and concerns
- Breastfeeding support
- Nutrition classes and support groups (at some locations)
- Providing personalized referrals to other programs and services
During your appointment, WIC will need to verify your identification, address, income, and pregnancy, if you are pregnant.

Proof of Identification
Bring identification for yourself and each person applying for or on WIC. Any of the IDs listed below are acceptable.
- Immunization Record
- Birth Certificate
- Hospital Record/Crib Card
- Health Insurance Identification
- Social Security Card
- Photo Identification
- Driver's License
- Student Identification Card
- State Identification Card
- Work Identification Card
- Pay Stubs
- Voter Registration Card
- DHS Identification (MiHealth card, Medicaid, SNAP)
- Michigan WIC EBT Card

Proof of Where You Live - Your Physical Address
Bring one of the following:
- Driver's license or Michigan ID
- Voter registration
- Utility or rent/mortgage receipt
- Passport/Visa
- Homeless shelter address
- Mihealth card (Medicaid)
Note: U.S. citizenship is not required for Michigan WIC eligibility

Proof of Income Eligibility
Receiving the following services make you eligible for the WIC program.
- Medicaid Programs
- Food Assistance Program (FAP)
- Family Independence Program (FIP/SNAP)
If you do not receive other DHHS programs, you must bring all sources of income. Examples include:
- Paycheck stubs dated within 30 calendar days of your appointment (example: weekly pay, bring four paycheck stubs)
- Child support (e.g. court papers of history of payments)
- Social security benefits statement
- Tax return IRS 1040
- Unemployment insurance benefit statement
- Retirement income statement
- If you receive financial support from someone who lives outside of your household, bring a signed and dated letter from the person or social service giving support with their name and the amount and type of support given.

- Written statement from a physician/health care provider
- Ultrasound documentation
- Physical appearance
- WIC referral form
