In-person WIC Appointment Details:
Please note the type of appointment you are scheduled for at the beginning of the text message you received. Provide the information below for the appointment listed in your text message before or during your appointment. Your WIC office can tell you how to share this information with them. Call the number in your message with questions.
What to bring for each type of appointment:

WIC Certification (CERT), Recertification (RECERT), or WIC Client Connect Certification (WCC) Appointment:
- Proofs Required:
- Identity
- Address
- Household income or proof of Medicaid
- If pregnant, proof of pregnancy
- Clients listed in text message.

WIC IEVAL (Infant Evaluation), CEVAL (Child Evaluation), or WEVALV (Woman Evaluation)
- Clients listed in text message.
- ID or WIC EBT Card.

Nutrition Counseling with Registered Dietitian (NCRD) Appointment
- Client(s) listed in text message.
- ID or WIC Card.
- During this appointment, you'll be able to discuss any questions that you may have with a Registered Dietitian and receive tips for healthy eating.
