Mandatory Reporting

It is mandated that all hearing test and screens on babies less than 12 months of age and all diagnosed hearing losses in children under three years of age need to be reported to the EHDI program (Act No. 31 Public Acts of 2006).

Reporting Early Intervention Status

EHDI provides referrals for early intervention for all infants identified as Deaf or Hard of Hearing. If the referred child is not known to you, please contact the audiology clinic to discuss ways to improve collaboration between sites. Please ensure that the assigned service provider completes and returns the Intervention Status Form, and that the date the Individualized Family Service Plan (IFSP) was signed is documented. This form allows us to verify that early intervention services were offered to the family. To obtain the Intervention Status Form, please contact garciam@michigan.gov.
