Michigan Office of Administrative Hearings and Rules for Michigan Department of Health and Human Services
The Michigan Office of Administrative Hearings and Rules (MOAHR) was created by EO 2019-06 as a Type 1 Agency within the Department of Licensing and Regulatory Affairs (LARA). MOAHR holds administrative hearings for a variety of departments when decisions are contested. The Benefit Services Division (BSD) holds administrative hearings for MDHHS related to Medicaid program eligibility/public assistance and, once enrolled into Medicaid programs, for beneficiaries and providers who are contesting actions taken by MDHHS, their contracted agencies, or managed care entities after internal appeals have been exhausted. MOAHR-BSD receives over 60,000 hearing requests per year regarding numerous Department programs.
Mission Statement
The MOAHR benefit services division mission is excellent customer service in the adjudication of administrative fair hearings. In pursuit of its mission, MOAHR promptly schedules and mails notifications of administrative hearings for persons requesting a hearing. MOAHR for MDHHS issues timely, clear, concise and legally accurate hearing decisions and orders.
Medicaid Benefit Hearings Forms
Several hearings related forms are accessible below for hearings related to the Medicaid beneficiaries and Medicaid providers. MOAHR forms are made available in Microsoft Word format so they can be downloaded and filled in using a computer. The forms can also be printed out on paper and completed by hand.

Public Assistance Hearings Forms
This form is used to appeal Michigan Department of Health and Human Services (MDHHS) determinations for the food assistance program (FAP), the cash assistance program (FIP), the child development grant program (CDC), the state emergency relief program (SER) and for applications for Medicaid, MIChild, and Healthy Michigan Plan, that were denied. This public assistance request for hearing form is different than the Medicaid form or the Medicaid managed care organization form which are found in the next paragraphs. Submit the public assistance request for hearing to your local MDHHS office using this general public assistance request form:
