Many countries require specific travel vaccines for visitors. International Health Regulations require that yellow fever vaccine be administered only at designated Yellow Fever Vaccination Clinics. In the United States, the responsibility for designating Yellow Fever Vaccination Clinics has been delegated to State Health Departments. Health care providers who wish to purchase and administer yellow fever vaccine must complete the Application to be a Michigan Yellow Fever Vaccine Provider and be approved by the Michigan Department of Health and Human Services (MDHHS) Division of Immunization. Submission of application does not guarantee approval.  Approval will be based upon provider location saturation and completion of program requirements.  Sanofi Pasteur will not allow the purchase of yellow fever vaccine by health care providers unless they are an approved Yellow Fever Vaccination Stamp holder. Yellow fever vaccine produced by Sanofi Pasteur is obtained directly from Sanofi Pasteur, the sole distributor in the United States. Michigan Program Requirements

The MDHHS Division of Immunization is working to recertify previous yellow fever vaccine providers.  Previous yellow fever vaccine providers must complete the yellow fever application. Upon completion and submission, a member of the MDHHS yellow fever team will email when the application has been approved and provide the next steps in the process.

 A physician, physician's assistant, or nurse practitioner (with prescription-writing privileges) can legally administer the Yellow Fever vaccine after a physician completes the Application to be a Michigan Yellow Fever Vaccine Provider and is approved as a yellow fever provider.

Anticipated yellow fever providers must complete the application in full and email to yellowfeverprogram@michigan.gov.  Any applications with missing or incomplete information will be returned to the applicant. Applicants are also required to attach a narrative description of the supervising physician's experience providing travel vaccinations and their reason(s) for applying to be a yellow fever vaccine provider.

Once the Yellow Fever Uniform Stamp is received and the International Travel Stamp Receipt form is returned to the MDHHS Division of Immunization, the site will be considered a designated Yellow Fever Vaccination Clinic in Michigan and will be eligible to purchase and administer yellow fever vaccine. It will be required to report all yellow fever vaccine doses administered into the Michigan Care Improvement Registry (MCIR).

Adverse Event Reporting 

Adverse events reported to providers after vaccination should be submitted to the Vaccine Adverse Events Reporting System (VAERS) according to state guidelines. Private providers should report all adverse events directly to VAERS and notify MDHHS.

International Certificate of Vaccination or Prophylaxis

  • International Certificates of Vaccination (ICVP): When travel related vaccinations (particularly Yellow Fever vaccine) are required, the administered vaccine(s) must be recorded on the International Certificates of Vaccination as Approved by the World Health Organization. This booklet should be kept with the recipient’s passport at all times.
