CSHCS INFO #17-2017 (2) Family Leadership Network Deadline Extended until August 25th (Re-Sending)
Wed 8/9/2017 8:43 AM
Thank you to everyone who has shared the Family Leadership Network opportunity. We have received a number of great applications. Due to a low number of applications in a few regions, however, we have extended the application deadline until August 25th, 2017. Please continue to share this opportunity widely with any families or youth that may be interested!
The Family Center for Children and Youth with Special Health Care Needs (Family Center) and the Michigan Family to Family Health Information Center (Family to Family) are looking for individuals to serve on our Family Leadership Network (FLN). This is a statewide leadership opportunity for family members with a child with special needs or a young adult with special needs.
In recognition of the importance of family involvement, the FLN was created out of the common need to obtain diverse perspectives from families and receive input on programs and special projects. Family perspectives are highly valued and contribute to a better understanding of experiences with healthcare and other systems in Michigan.
We are looking for individuals who have personal experience as a parent, guardian or caregiver of a child or young adult (up to age 26) with a disability or other special needs; or be a youth (18 or older) with a disability or other special needs. We are seeking two members from each of the Michigan Prosperity Regions. If you are interested in volunteering on the FLN please review the following attachments.
- FLN Information Packet (FLN Position Description, FLN Operating Guidelines, FLN Reimbursement Policy & Form, Map of Michigan Prosperity Regions)
- FLN Application
After the August 25th deadline, applications will be accepted on a continuous basis. If you are unable to participate at this time but you know a parent or youth who might like to join, please send this information to them.
Questions? Please call Lisa Huckleberry at 517-335-0407 or Kristen Hawkins at 517-324-7396.
Thank you,
