Dear LHD Colleagues,
The attached 2021 Annual CSHCS consumer satisfaction survey and letters (single PDF) have been sent to a random sampling of CSHCS clients/families. If a family calls regarding the legitimacy of the survey, please advise them the survey is legitimate and encourage them to fill it out and return it. Please let me know if you have any questions.
