The Family Center is extending the deadline to April 7, 2018 for the LHD Small Grant application to be postmarked and mailed. This extension is to allow for any extra time that may be needed to complete the application.
Dear Colleagues,
The Family Center for Children and Youth with Special Health Care Needs (Family Center) and Children’s Special Health Care Services (CSHCS) are pleased to announce a small grant opportunity for the 2018-2019 fiscal year (10/1/18 – 9/30/19). Each Local Health Department (LHD) is eligible to apply for up to $10,000 for their project. Please see the attachment for the application and guidelines.
The objectives of the grant include:
Educate the community and target audiences about the Children’s Special Health CareServices and the Family Center’s program andbenefits
Increase family support and involvement through implementation of family-centered activities in order to increase knowledge and care for children with special health care needs and their families
Obtain feedback regarding improvements in the systems of care serving families, including LHD policies and procedures
The deadline to have the application postmarked and mailed is Friday March 30, 2018. Please note this is a fillable PDF application.
