Dear LHD Colleagues,
Attached is the new Payment Agreement Guide effective 4/1/2019. CSHCS Payment Agreements processed 4/1/19 or later will be calculated per the new rates. The new guide will be included in April’s renewal letters and will be mailed in SLAPP Packets beginning April 1, 2018. The Guide is also available on the CSHCS website by and the LHD SharePoint site.
NOTE:
The LHD Forms repository under LHD Forms Download in the CSHCS Database as well as the LHD SharePoint site and the CSHCS website will be updated as soon as possible.
