Applying for Medical Assistance:
To Apply for Medicaid, MIChild, Healthy Kids, Plan First! or other Medical Assistance Programs:

- Apply on-line or print, complete and mail the DCH-1426 form
- You can call or go to your local Department of Health and Human Services (MDHHS) to get an application.
- Then mail or return the Application to your local MDHHS.
- The MDHHS will determine your eligibility for Medical Assistance.
- The Department of Health and Human Services (MDHHS) has implemented a pilot policy for MDHHS administrative hearings in three counties. Medical Assistance (MA) hearing requests based on eligibility factors are included in this pilot. More information may be found here in MDHHS Bulletin BPB 2013-007.

MARS is a pre-screening tool that allows you to find programs offered by the State of Michigan that may help families in need with assistance for medical, nutritional, food, day care, temporary cash or other expenses.

Q: What is Medicaid? A: Medicaid is a health care program for low-income families & individuals. This includes families with children, pregnant women, and persons under the age of 21. It also offers help to persons who have a disability, and people age 65 and over.

Q: If I am not enrolled in a Medicaid Health Plan, where can I find out which doctors, pharmacy, hospital, or other medical providers that participate straight Medicaid? A: Contact your local Health Department. Your health department might have a list of medical providers that participate with straight Medicaid. Or call the medical provider. Maternity Outpatient Medical Services (MOMS) Questions and Answers:
Q: What is the MOMS program? A: The MOMS program provides immediate health coverage for pregnancy related services only. The MOMS program is available only for pregnant women while a Medicaid application is pending.
Q: What medical services does MOMS cover? A: It provides pregnancy related services during the pregnancy and through 60 days after the pregnancy ends. It may also provide for labor and delivery services (including all professional and inpatient hospital services).

Q: What is Medicare? A: Medicare is a Federal Health Insurance Program. The Centers for Medicare & Medicaid Services (CMS) administers this program. CMS is a Federal agency within the U.S. Department of Health and Human Services.
Q: Who might be eligible for Medicare? A: People 65 years of age and older, some disabled people under 65 years of age, and people with End-Stage Renal Disease (permanent kidney failure treated with dialysis or a transplant).
