Beneficiaries

  • Getting out from a jail or prison can be hard and cause a lot of stress. You may ask yourself:

    • Where will I live?
    • How can I get food?
    • How can I get to where I need to go?
    • How can I get help seeing a doctor or dentist?
    • How can I get my medications?
    • How can I get a job?

    The TCM program can help answer these questions and get you the help you need once you are free from jail or prison. The TCM program can help connect you with:

    • Doctor Visits
    • Dentist Visits
    • Mental Health Care
    • Substance Use Disorder Care
    • Medication
    • Food
    • A Place to Stay
    • A Ride
    • A Job or School
    • Other Needed Services

    The TCM program can help you move back into the community while you recover and get back on your feet. You will also receive all other Medicaid-covered benefits.

  • Yes, you may if you:

    • Are 18 years old or older;
    • Were just released from a jail or prison; and
    • Have a medical, dental, or mental health condition, or a problem with alcohol or drugs.
  • You can get TCM services for up to 12 months. You do not have to use the TCM services and you can leave the program any time.
  • You can talk with your jail or prison corrections officer, or parole/probation officer, about getting TCM services if you are getting out soon. You can also talk to your parole/probation officer if you just got out. They can help to set up a meeting with someone from the TCM program to talk with you about what services you may need when you get out of jail or prison.  You may also call the Beneficiary Help Line at 1-800-642-3195 or TTY 1-866-501-5656 for more information.
  • You may get TCM services from one of these providers:

    • Community Mental Health Services Program
    • Federally Qualified Health Center
    • Rural Health Center
    • Tribal Health Center
    • Tribal Federally Qualified Health Center
  • You will get TCM services from a case manager who may be a nurse or social worker. The case manager may work with others who will help you get your services. Others who will help you may be a community health worker, peer support specialist, or a peer recovery coach. Other members of the TCM support team may include a physician, physician assistant, nurse practitioner, or a clinical nurse specialist.
  • Your TCM services will end after one of the following happens:

    • You have completed 12 months of the program
    • You have received the maximum number of services that are allowed
    • You don’t have Medicaid anymore
    • You don’t want the TCM services anymore
  • When you get out, your year of services will start over, but the number of services you get will not. Whatever you had left the first time is what you will have when you get out the second time. If more services are needed, the TCM provider will need to ask Medicaid for more.
  • The policy does not prohibit those who have a history of incarceration from participating as a TCM provider. These individuals may become employed as a community health worker, peer support specialist (PSS), or a peer recovery coach if they meet the application approval process for the specialized training and certification requirements.
