County Jails and Prisons

  • You may identify an inmate who is soon to be released from your facility or is currently being released if the individual has any chronic or complex physical or behavioral health care need, and they are 18 years of age and older.
  • The inmate should be informed that by participating in the TCM services program they will have access to professionals who will help them to transition back into the community by working to coordinate the services they will need once they are released back into their community. For example, by participating in the TCM program they will receive help in gaining access to housing, food, transportation, doctors, prescription drugs, finding a job, and other supports that they may need. They will also receive all other Medicaid-covered benefits.

    Inmates may also be provided a TCM brochure and be directed to the TCM Services Benefit Program webpage. 

  • Providers that have been approved to provide TCM services are listed with their contact information on the TCM Services Benefit Program webpage.

    Providers must be approved by MDHHS as a Medicaid TCM provider. They must attest that they have the capacity to provide all of the core elements of case management services. The TCM provider may be a:

    • Community Mental Health Services Program (CMHSP)
    • Federally Qualified Health Center (FQHC)
    • Rural Health Center (RHC)
    • Tribal Health Center (THC)
    • Tribal Federally Qualified Health Center (Tribal FQHC)
  • While the in-reach component is highly recommended, it is not a requirement for the in-reach to visit occur. The in-reach component is where a designated professional from the TCM facility will visit with the beneficiary prior to their release to further discuss the details of the TCM services benefit. However, it is understood that due to certain security standards an in-person visit may not be allowable. A visit via video conference is another option to utilize when an in-person visit is not the best option.
  • The in-reach visit should occur at least 7-14 calendar days, and no more than 30 calendar days, prior to the inmate’s release.

    The TCM provider will plan for the inmate to meet either in-person or via videoconference with a case manager or other representative prior to the inmate’s release. The case manager may be a nurse or social worker and the other representative may be a community health worker or other peer support professional.

  • For inmates being released from a prison, their benefit plan should automatically switch to the TCM benefit plan within the next 30 days.

    For inmates being released from jail, the corrections officer may contact the local DHHS office to ensure the Living Arrangement page within Bridges is correctly updated to end the program enrollment type (PET) code of “INC-Jail.”

    Jail or prison corrections officer may also contact MDHHS Provider Support (providersupport@michigan.gov) to end the Medicaid incarceration benefit plan restrictions.
