In January 2011, Michigan was selected by CMS as one of fifteen states to be awarded a contract for the development of an integrated care plan for individuals who are dually eligible for Medicare and Medicaid. The goal of the demonstration is to improve quality and access to care by more effectively aligning the two programs and bridging the divide between the physical health, long term care, and behavioral health systems.
After conducting an extended stakeholder engagement process, MDCH submitted a proposal to CMS in April 2012 outlining the structure and goals of its integrated care plan. Through conversations with CMS and stakeholders, MDCH continues to develop the integrated care plan and related Memorandum of Understanding (MOU) and Request for Proposal (RFP).
The following documents highlight the stakeholder process and current structure of the demonstration and include the original proposal, a FAQ, a detailed explanation of the Care Bridge, and a PowerPoint presentation on the project.
