Letters of Support for the National Interest Waiver (NIW) Program
The Michigan Department of Health and Human Services provides letters of support for National Interest Waiver Petitions.
To receive a letter of support the employer must fill out the request form, print it on the employer's letterhead, and sign it.
Please also include the following:
- A copy of the employment agreement or contract.
- Documentation indicating that the employment site is in a federally designated shortage area (HPSA or MUA/P). In the case of multiple work sites, please list them separately.
- Search for HPSA and MUA/P designation by address through HRSA Geospatial Data Warehouse: http://datawarehouse.hrsa.gov/GeoAdvisor/ShortageDesignationAdvisor.aspx
Michigan does not provide letters of support for physicians with prior service in Michigan without a minimum of 1 year further commitment in a Michigan underserved area.
