Everyone in the MI Coordinated Health (MICH) plan will have a Care Coordinator. The care coordinator will work with you to assess your needs, make a care plan that addresses those needs, and helps you coordinate with referred providers. Your care coordinator will help you get the services listed below if they are right for you.
