Electronic Home Office Cost Statement Form Update: This update file can be installed only after installation of the Michigan Electronic Home Office Cost Statement from the Michigan Home Office Cost Statement CD. Electronic form used by Long Term Care Providers operated or managed by a Home Office entity, to report costs of Home Office entity.
