Note - You need Microsoft Word or a compatible word processor program to view the ".doc" files.

Note - Cost Report templates have been tested with Microsoft Office 97, 2000 and XP 

Medicaid Electronic Provider Cost Report Application, applicable to 2016 and forward.  Applicable to cost reporting periods that begin on or after March 1, 2015.  Medicaid LTC Providers can assess Application from DCH-File Transfer Application.  Non-Medicaid LTC Providers must contact this office for a CD containing the Application
